FEES AND PAYMENTS

Fees
Dr. Cheri Hodges will complete a physical examination during your initial 90-minute consultation to get an understanding of your concerns before providing her recommended care plan and offering service. The fee for the initial consultation is $225.00. Each follow-up appointment is 45-minutes and costs $175.00.

Payments
Hodges Physical Therapy operates on a fee for service basis and accepts checks, credit card and direct online payments through Zelle. Payment is required at the time services are provided.

Canceling an Appointment
If you cannot keep your appointment, please notify Hodges Physical Therapy at least 24 hours in advance. If you will be unavoidably late for your appointment, please call 480.849.2437. If you arrive later than 15 minutes, Dr. Hodges may need to reschedule your appointment.
